- Fix duplicate taskId extraction in claude.ts
- Fix tool name references (mcp__acp__ReadPlan/WritePlan)
- Move require('fs') to top-level import
- Add type safety for _meta field with NewSessionMeta type

if i had more time, i'd want a bit more focus in the prompts on it creating a real todo list and ticking things off - it's somewhat haphazard atm
